IVF – when it doesn't work naturally

IVF (in vitro fertilization) is often recommended if previous inseminations have been unsuccessful, the fallopian tubes are blocked, or other conditions such as PCOS or endometriosis are present.

You will be supported with hormones that you inject yourself at home. The aim is to mature several eggs instead of just one, as in the natural cycle. We closely monitor your cycle with ultrasound and blood tests. As soon as the right time has come, we trigger ovulation in a targeted manner.

About 36 hours after ovulation is triggered, the eggs are retrieved under short, gentle anesthesia. In the laboratory, your eggs are then placed in a nutrient solution with the prepared sperm – either from your partner or a donor.

In a special incubator that mimics the conditions in the uterus, fertilization can take place within the next few hours.

Once a well-developed embryo has formed, it is carefully transferred to your uterus a few days later using a fine catheter – a usually painless, short procedure without anesthesia.

ICSI – when the next step is necessary

ICSI (intracytoplasmic sperm injection) is often recommended when fertilization has not occurred during IVF or when sperm quality is severely impaired.

The procedure is similar to IVF: you start with hormone stimulation so that several eggs mature. These are then retrieved in a short procedure under local anesthesia.
The key difference lies in the fertilization:

Instead of leaving the sperm and eggs to fertilize on their own, ICSI involves injecting a single sperm directly into the egg using a fine glass capillary – under a high-resolution microscope and with the utmost precision.

After fertilization, the embryo develops as in IVF until it is transferred to your uterus a few days later.

TESE – Sperm retrieval directly from the testicles

In rare cases, repeated attempts to obtain a semen analysis show no sperm in the ejaculate – for example, in cases of obstruction of the vas deferens or after sterilization. But even then, there is hope: with a procedure known as TESE (testicular sperm extraction), it is possible to retrieve sperm directly from the testicular tissue. In a small outpatient procedure under short anesthesia, a urologist removes a tissue sample from the testicle. In many cases, this tissue contains sperm capable of fertilization. The sperm obtained is frozen and will be available to you later for ICSI treatment.
