Here's how it works

Please fill out our medical history form and send it to us by mail or email along with all the necessary documents. Once we have received all your documents, we will contact you by phone as soon as possible to schedule an appointment for your initial consultation. Please understand that processing may take up to a week.

Important information

  1. Are you over 40 or interested in social freezing?
    Then, in addition to our medical history form, we will need the following blood values from you: E2, LH, progesterone, FSH, and AMH. Please have the values taken between the 2nd and 5th day of your cycle by your gynecologist and send us the results together with the medical history form and the subject line “Initial consultation appointment” to the following email address: mfa@endlichnachwuchs.de
  2. Do you have any previous findings related to your desire to have children, e.g., sperm analyses or surgical reports? Please also send these with the subject line “Initial consultation previous findings” to: mfa@endlichnachwuchs.de

During a treatment cycle, it is crucial to determine the exact timing of ovulation or the trigger point. While an ultrasound provides important visual information, it only offers an estimate. Blood tests, however, allow us to measure precise hormone levels. To monitor how these levels develop, it is necessary to take blood samples several times throughout the cycle—ideally on the same day as your ultrasound.
